2011-2012 SUMC Youth Group

Every youth needs to be registered in order to participate in any Youth Group activity. Each youth will be allowed to join us only one time without registration. Please take a moment to complete the information below and send it back with your child. Thank You! Donation Request - In order to provide snacks for Club 56 participants, we are asking that you send $10 with your child to cover the snacks that we will serve each Wednesday to your "starving" child.
